(WNY News Now) In order to determine the region’s health needs and shape upcoming wellness initiatives, the Chautauqua County Health Department is asking locals to participate in a crucial survey.
Mayville, New York To learn more about the needs, difficulties, and present state of health in our county, the Chautauqua County Health Department is undertaking a Community Health Assessment. The data supplied will be used to create a Community Health Improvement Plan that will guide the allocation of resources.
By completing the Chautauqua County Community Health Survey, all county residents are urged to take part in this effort. One excellent approach to get involved and contribute to bettering the health of your community is to complete this survey. Since you live here, you are aware of the advantages and disadvantages of our community; your ideas and opinions are valued!
It takes 5–10 minutes to complete, and answers to the health survey questions are kept private. The survey will be accessible until March 31, 2025. Respondents to the survey can enter to win one of five $50 Tops Gift Cards in a random lottery by providing contact information that is unrelated to their survey responses.
